On andriod. After choosing subscribe and save. The options to pay page will load. Saved credit cards, mile points, ect. In blue the last option is "gift card, voucher, promo code". Click that. New page will load with promo code box faded out at the bottom. There will be a slider to click on the right side. The box will open up to type/paste the code. Once typed an arrow will appear on the far right of the box. Click it to enter the code.

Host Defense is probably the most reputable. Anything this cheap is almost certainly of low quality.
It's really, really easy to grow lions main and other beneficial mushrooms if you invest a couple of hours reading up on how to do it.
